Getting Started

How to Request Care

To request care, you must fill out a Waiting list request form, DD Form 2606PDF File, and submit to either CDC in person or by fax. Gibson CDC DSN fax 246-6886, Comm 846-6886 or Maxwell CDC DSN fax 263-5566 or Comm 853-5566.

All dependents, ages 6 weeks to 5 years, of active duty military/DoD civilian employees may use the CDCs in accordance with Air Force priorities. Bona Fide houseguests of eligible patrons may use the CDCs on a space available drop-in basis. The CDCs operate at capacity during some periods; therefore, space may not always be available for all children.

Based on guidance from AFMC and Department of Defense Instruction 6060.2 dated Jan 93, the following priority for child care is provided Children of:

  1. Active duty military personnel and DoD civilian; single parents
  2. Active Duty Military and DoD civilians- Dual employed parents
  3. Active Duty Military/DoD Civilians with non-working spouses (one income family) assigned to the installation
  4. Active Duty Military/DoD Civilians with non-working spouses (one income family) not assigned to the installation
  5. Retired Military; single parent
  6. Retired Military; dual employed parents
  7. Retired Military, one parent employed (one income family)
  8. Guard & Reserve: However, entitled to priority 1, 2 and 3 while on active duty orders assigned to Kirtland AFB. If Guard and Reserve members’ active tour of duty is impending, our local policies allow them to sign up early and be planned on a waiting list. The appropriate paperwork must be completed in advance and once active duty orders are received by the childcare facility, the Guard or Reserve member will move to priority 1, 2, or 3 lists accordingly
  9. DoD Contractors

Care of children with physical, mental, or emotional disabilities or those requiring special diets, special procedures, medication for chronic illness or other special attention - cannot be cared for or registered on the waiting list, without referral to the CDC Director or designee, for evaluation.

Registration Paperwork

When a slot is available for your child, the CDC staff will set up an appointment at that time and will provide information on the facilities, give you a tour and provide initial orientations. If you drop in without an appointment, the staff will be happy to provide you with information on registration but may not be able to accept new registrations or re-registrations due to daily in-processing and out-processing of established patrons.

To register in the program, you must bring:

  1. DoD identification to establish eligibility
  2. Your child’s current immunization records to the center.
  3. A current leave/Earnings Statement and or pay slip for every adult member of the household.

CDC will provide forms for you to fill out:

  1. A health evaluation for your child within 30 days of enrollment, in order for your child to receive care
  2. An AF Form 1181 (Air Force Youth Flight Program Patron Registration)PDF File must be completely filled out and signed for each eligible child a completed form includes a local emergency contact, other than the sponsor and his/her spouse, their phone number, authorization for excursions/field trips, and proof of up-to-date immunizations

You will also be provided a copy of our Parent HandbookPDF File to review prior to your child starting our program.

If an AF Form 1181 is not completed within 10 days, it will be destroyed.

A completed AF Form 357 (Dependent Care)PDF File must be provided for all single and dual military sponsors before child care can be provided in the CDCs.

It is your responsibility to ensure immunizations are up to date or child care may be denied. You must bring each child's immunization records to the center for verification. In accordance with AFI 34-248, Child Development Centers, all children must receive immunizations recommended by Air Force policy. Failure to do so could cause loss of the available slot for your child.

All records are kept Confidential and will be maintained at the front desk and only those staff with a need to know will have access to them.

Certified written notification is required for any special care a child may need, any food substitutions, or any allergies a child may have prior to your child receiving care.

The entire registration package must be completed in its entirety prior to receiving childcare.
